Connect with a Member Enrollment Specialist.
When a Member Enrollment Specialist contacts you, you’ll connect with a caring professional who will always put your needs first. They’ll tell you about the program. Show you how it works. And start you on a personalized path to success.
We’re here to listen.
Step 2
Get hands-on support from a Care Coach.
After you join the program, you’ll work 1:1 with a dedicated Care Coach who will help you overcome barriers to care and achieve your goals. They’ll work with you to create a personalized care plan. Find and schedule time with health professionals. And follow up after each visit.
We’re here for you every step of the way.
Step 3
Access treatment with health professionals.
As needed, you’ll get time with behavioral health therapists and psychiatrists through in-person or telehealth visits. They’re an extension of our integrated care delivery team.
We’re here to ensure quality access for every member.
Step 4
Graduate with confidence—and true peace of mind.
When ready, you’ll graduate equipped with the tools and know how to manage your health and access care when you need it. You’ll be regularly visiting a primary care provider and on a path to long-term health and wellbeing.
